Course 2

Process Safety Management (PSM)
This course on process safety is the Only One that shows how each process safety element either controls or compensates for human factors. It teaches not just the OSHA PSM elements, but also the elements that are missing such as Human Factors (controlling human-system interface, alarm management, miscommunication, human fitness-for-duty, work-place environment, stress), Leadership/Accountability, and Major Projects Considerations (how to deliver process safety by the end of a major project). The course also correlates these elements to requirements in Risk-Based Process Safety, authored by CCPS/AIChE, 2007. Only PSM experts from PII with 20+ years of experience in hands-on process safety implementation teach this course.

Course 7

Management of Change (MOC) and Pre-Startup Safety Review (PSSR)
This 2-day course explores proven strategies for implementing a workable MOC system and customized approaches to fit your company’s culture. It also shows how to implement physical and instrumentation changes through the proper use of pre-startup safety review (PSSR) systems, sometimes called operational readiness reviews (ORRs).

Updating and Revalidating Process Hazard Analyses (PHA)
This course prepares you to execute a PHA revalidation that complies with PSM regulations and industry standards. PHAs usually must be updated and revalidated every 5 years. Several factors determine the extent of the revalidation analysis, including the quality of your previous PHAs and its documentation, process and facility changes, incidents during the past 5 years, and new interpretations of codes and standards. In some cases a complete “redo” may be required if the previous PHA quality and documentation did not meet your standards or regulatory requirements.

Course 10

Human Error Prevention
Human error is widely acknowledged as the major cause of quality, production, and safety risks in many industries. This course explains the underlying reasons why humans make mistakes and how you can prevent these mistakes. It also explains the lower limits of human error that are achievable, explains coupled or common-cause human error (which depends heavily on task design), and reviews various ways to error-proof tasks. The course provides an overview of Habits and how to lower the rate of this class of errors. And finally, the course covers how to predict human error and how to find the root causes of human errors. Although it is unlikely that human error will ever be completely prevented, there is growing recognition that many human performance problems stem from a failure within organizations to develop an effective policy for managing human reliability.

Incident Investigation/Root Cause Analysis Leadership
The instructor was a primary author of the AIChE/CCPS textbook Guidelines for Investigating Chemical Process Incidents (2003). The methods, which are based on the most widely used approaches as developed by US DOE, are for your use License-Free. Methods include best practices for gathering data, causal factor charting, 5-Whys, fault tree, and use of a comprehensive Root Cause Chart. Other companies may teach similar methods, but they charge a license fee to use the methods. No need to pay licensing fees for methods that are free to all. More than 2000 Investigation Leaders in the region have attended this course. Many companies have replaced using other methods with these more efficient (yet still comprehensive) methods. Course 6

Mechanical Integrity
This 2-day course prepares you to evaluate your existing practices versus recognized codes and standards and then to efficiently improve or develop and implement your mechanical integrity (MI) program. Topics also include how to merge your MI program with a reliability program. This course is unique in providing you a documented starting point for the inspection, test, and preventative maintenance (ITPM) for each critical component in a process; while also providing the performance values, such as failure rates or probabilities of failure on demand, for each component, if they are maintained as stated in the course.

Compliance Auditing for Process Safety/Risk Management
This course covers every aspect of auditing from gathering data via records and interviews, keeping notes, report writing, and making recommendations. Using all rules and methods taught in class, the second day is a workshop to audit actual PSM practices in a real-time setting.

Course 12

Safety Instrumented Systems (SIS/SIF/SIL)
Learn the basics of SIS, including issues related to specific and systemic errors (such as humans leaving the SIF bypassed) that must be accounted for in SIL Verifications for the PFD to be valid. Learn the design fundamentals for SIF. Over 5 exercises, gradually learning how to perform SIL Verification Calculations, the right way. Finally learn the critical maintenance practices that MUST be followed to achieve high SILs in practice, yet 90% of the companies using high SILs do not understand or implement these maintenance practices. This course is unique in that it illustrates the rightly place of SIFs in the hierarchy of IPLs and it demystifies the complicated concept of SIL verification and SIF maintenance. Taught by experts (William Bridges or Art Dowell) who are certified in functional safety and who helped to shape the original SIS standards.

Course 5

Writing Effective Operating and Maintenance Procedures
This 2-day course teaches how to write effective step-by-step operating procedures and how to develop troubleshooting guides from PHA documentation. This is very much a “How-To” course, designed to teach skills. The first day of the course is for both operations and maintenance personnel; the second day is mainly for operators (since they typically must write more in-depth troubleshooting guides). This course is unique in that it teaches best practices for how to format a page and structure a step to achieve optimum clarity, while also teaching how to ensure the steps are accurate (the content is correct).
