The MHA method is intended to address the four major hazards usually of concern in process safety and their impacts on people and the environment. However, some PHA practitioners apply PHA methods to other types of hazards. Therefore, in an extension of the MHA method called Direct Hazard Analysis (DHA) other hazard types are addressed.
|
The HAZOP method requires PHA teams to consider a variety of deviations from design intent and brainstorm causes of the deviations. Unfortunately, it is difficult for PHA teams to select only those aspects of design intent that will result in the identification of issues within the scope and objectives of the PHA. Consequently, effort is often expended in HAZOP studies on issues that turn out to be unimportant. MHA starts with the direct identification of pertinent initiating events for hazard scenarios.

There are two main aspects of managing quality on a project. It all starts with understanding the customer's expectations for quality. You then need to put a plan in place to meet the customer's expectations.
|
The first step which should be taken is to establish the objectives of the HAZOP study.
Discussions with management should clearly establish these objectives. Sufficient time should be spent defining the objectives in writing and obtaining management’s approval
|
The HAZOP technique is a highly disciplined procedure
|
The purpose of this Project HSE Plan is to define the project HSE management system; detail the Project HSE objectives and expectations; and set out the related activities, plans, procedures and resources required to meet these.

Fire Protection Analysis
The scope of the FIREPRAN REVIEW was to identify and assess the Fire & Explosion hazards associated with the operation of the BS2 facilities

There's still time to get a seat in this very popular course. This course is taught by PHA leaders who, collectively, have been and are currently involved and managed 12,000+ hazard evaluations (using all qualitative methods: HAZOP, What-if, FMEA, and Checklist) of entire units. Some are also contributing authors of the 2nd and 3rd editions of the Guidelines for Hazard Evaluation Procedures, CCPS/AIChE.
